I pay BMO $149 per year for the "priviledge" of having a US dollars credit card. There are no points or other perks attached (like other gold cards where there is a yearly fee), simply the ability to use it without foreign exchange penalty.

Not only that. Every time I want to pay for my US dollars Mastercard, I must get a "live" person to do the transaction. A pain in the butt when every other bill I pay can be done on-line.
